I would start by talking to the parents about what they are doing to work with the child during the week. Ask if he attends to anything for longer than 15 seconds. If the child has attention issues, actitivies can be completed quickly so that he/she maintains attention. Try a variety of acticities: singing, walking, apps on your phone (but don't ever let them hold it!), bean bag toss, or just dumping items and having him pick them up. Try sitting the child in a high chair during sessions. Try doing a puzzle where the child has to walk across the room to get the pieces to help with motor movement.
Keep in mind it could be more than just attention. It could be related to lack of discipline. The child will also need to do a lot of following directions.
