I have this same exact problem and this is what I've done. I targeted final sounds first. So oof instead of foo and then worked it to medial oofoo and then slowly worked off the beginning vowel. It's helped with a couple. I seem to have several that have difficulty with adding a sound in when we move to syllables and I tried all the same things as you.
