You can try using a recorder with a lapel microphone. I just tested a rather soft spoken child and clipped it to his collar. It came out pretty clear. Prompting him to look at me so I could read his lips and watch his tongue etc helped too.
I bought my microphone at Radio Shack for $26 dollars. They called it a Tie microphone, I think.
