Tongue thrust in a 4 yr. old

I tested a 4 year old girl who presents with age appropriate language skills. She has a pronounced tongue thrust (open mouth at rest with tongue protruding). Mom insists she needs direct speech therapy and had pediatrician write a note recommending direct speech therapy. How can I convince mom that this is a structural/medical issue when she has the pediatrician saying otherwise?
