I agree with speechc that you have to tread very lightly. I am in a clinic setting and I have evaluated and treated many three and four year olds where the parent was focused on the jargon, and dismissed everything else (He has a lot of energy! He like to play around! He's just shy!). If there is a team, then you want to of course coordinate with others about how to bring up your concerns. If the child is only receiving speech, I would gradually bring up things you see in sessions and steer the parents towards having the child evaluated by a child pyshcologist or developmental pediatrician, or tested for special needs preschool. I tell them that a child of that age should have better eye contact, ability to follow directions, attention, motor skills, etc. I focus on things that are not related to speech output and suggest that their child could benefit from additional services. Emphasize that you want to make sure that the child gets all the help that he can well before it is time for K.
