What does he do in therapy? Does he attend? Is it clear that he understands what you are expecting him to do? If all else fails, I think I might try actually moving his mouth, and reinforcing that. Hard to motivate a child when you don't know what it truly reinforcing to them though. Maybe let the child lead you, let the child have some options and find what he loves to play with. Then take his spontaneous speech and model correct productions....and begin holding his favorite toys/snacks/and making him imitate you before he gets anything at all.
