I would look at what he can do and build from there. When I start with any child with language delays, I figure out which age range their skills are at. If I see that they are doing several things that a child does between 12 - 18 months, then I work on other skills from that age range. So if the child is functioning at a 12 month old level, then I focus on what a 12 months old needs to learn next.
You might be starting at a very basic level with him - imitation of gross, fine and oral movements and sounds/words, attending, pointing to named objects, indication of named clothing items and name parts of body. Those are all things I would work on with a child who is functioning at the 12 - 18 month level. You might have to try some different things out and see what he is interested in or willing to do.
