It's tough at this age to tease out underlying causes... in the mean time, does he have a functional communication system in place - sign, pictures? If not, I would work that into your therapy to reduce his frustration and get an idea of what kind of true communication abilities he has currently. Work on earlier sounds like /m,b,p,h/ in repetitive syllables - this can go really slowly with apraxic kids!
