Is there anything he can focus on for a longer period of time (movies, songs, play activities...)? If so, I would suspect something other than true ADHD. I would probably write a goal about task completion to address attention, because it's easier to chart - i.e. will complete x# of teacher-directed tasks during a session - and then use a picture schedule and some sensory tools (fidget toys, blow-up seat cushion, stretchy tubes around the chair legs, etc.) to support him. The other thing to consider is how much his receptive language delay is affecting his attention - it's hard to focus on something that doesn't make sense!
